Continual Discharge Planning

Discharge planning begins in the first week of treatment. The development of this plan begins during the initial assessments made by the multi-faceted treatment team. The team of substance abuse experts constructs the treatment plan much like the continuing care plan with the goal of the individual to be able to maintain the same mental balance after discharge from treatment. To do this each member of the treatment team is responsible for the formulation of a specific segment of the plan.
